武警二院专家大剖析：胃癌治疗方法

　　胃癌是我国常见的恶性肿瘤之一，在我国其发病率居各类肿瘤的首位。在胃的恶性肿瘤中，腺癌占95%这也是最常见的消化道恶性肿瘤，乃至名列人类所有恶性肿瘤之前茅。目前胃癌治疗的主要方式以手术，放化疗为主要手段
　　是我国常见的恶性之一，在我国其发病率居各类的首位。在胃的恶性肿瘤中，腺癌占95%这也是最常见的消化道恶性肿瘤，乃至名列人类所有恶性肿瘤之前茅。目前的主要方式以手术，放化疗为主要手段。患者在接受这些治疗方法时需对这些常规的治疗法需要一定的了解，这样才能有效根据自己病情，采用适当的方法配合医生治疗，达到治疗目的。
　　1、 手术
　　胃癌手术分为根治性切除手术、姑息性切除手术、短路手术。根治性手术又分为：有根治性切除和扩大根治性切除两种术式。
　　手术其最根本的目的是切除胃癌病灶体。胃癌原发病灶体采用根治切除(胃大部切除术)较容易，能起到良好的治疗效果。但对于病灶才用手术却很难达到根性目的。因为肿瘤的基本组成的细胞很小需要显微镜才可以看到，一个火柴头大小的细胞群体有3000万细胞，为了描述的方便，我常常比喻如同水泥粉末一样。这些细胞可以如同特种兵或者成为师团级建制的成长，到了一定的时间或者空间又游移到别的地方形成为新的病灶。因此手术对于远处转移病灶体是很难达到治疗效果。
　　2、放射治疗
　　放射并发症较多，甚至引起部分功能丧失;对于晚期肿瘤患者，放射治疗效果并不完好。同时体质较差，年龄偏大的患者，继续放疗只能导致虚弱的生命更加垂危，加速了患者死亡。胃腺癌放射敏感性低，单独放疗或与化疗综合治疗后肿瘤缩小50%以上的只占60%，肿瘤完全消失者仅10%，因此，胃癌不能单独用放疗来根治，放疗在胃癌治疗中的作用主要是辅助性的或姑息性的。多用于综合治疗，放疗的主要形式有术前放疗、术中放疗、术后放疗和姑息性放疗等四种。据文献报道术前放疗可使根治手术切除率提高2%左右，使中晚期胃癌5年生存率提高1%～2.5%。
　　3、 化学治疗
　　化疗不同于手术治疗和放射治疗在于它对人体的治疗中的整体性，通过口服及静脉给药在全身起作用。而，正是一种全身性疾病的局部表现。对病人的最大的威胁是扩散和转移。化学治疗对于消灭某种的远处转移或防止复发，有其独到之处，是癌症治疗方法中不可缺少的组成部分。
　　化疗是近年来在肿瘤治疗中进步最快的治疗方法。但是化疗药物常&是非不清&、&敌我不分&，在杀伤肿瘤细胞的同时，也杀伤了人体正常细胞。化疗可能出现如下的毒性作用和不良反应：
　　a、静脉注射化疗药物时，操作不慎药液外漏可引起局部组织坏死和栓塞性静脉炎
　　b、抑制骨髓造血系统，主要是白细胞和血小板下降。
　　c、可不同程度的损害肝脏细胞，出现谷丙转氨酶增高、胆红素上升、肝肿大、肝区疼痛、黄疸等
　　d、有些化疗药物对心血管系统有毒性作用，严重的可发生心力衰竭
　　e、对呼吸系统有毒性作用和不良反应的化疗药物可引起急性化学性肺炎和慢性肺纤维化，甚至出现呼吸衰竭
　　f、泌尿系统的毒性作用和不良反应表现有蛋白尿，少尿或无尿，有的发生血尿
　　j、某些药物可影响生育，导致畸胎
　　h、出现恶心、呕吐、脱发等不良迹象
　　4、DC-CIK细胞疗法
　　这是目前在国内外具有先进水平的肿瘤方法。此项技术由京城首家引进。该疗法利用采集分离肿瘤患者自身的有核细胞，用多种细胞因子在体外诱导并大量扩增，使其活化为特异的杀伤性免疫细胞，能使其对肿瘤的杀伤活性呈数倍至数十倍的增长，然后将其回输给肿瘤患者，以提高机体特异性的抗肿瘤能力。同时调节和增强机体的免疫功能，作为癌症的重要辅助治疗手段，弥补了一些癌症对放疗、化疗药物不敏感的缺陷。该疗法不受癌症组织类型的限制，因此对大多数癌症均有杀灭作用。在配合手术切除、介入、射频、氩氦刀等治疗和降低肿瘤复发率、转移率等方面具有积极作用，且没有通常放疗、化疗后的明显毒副反应。

　　胃癌晚期不能手术还有别的方法吗，临床上由于胃癌早期症状不明显，且临床发展速度快，许多患者在确诊的时候病情就已经进入了中晚期，这时候如果不能及早的发现将随时面临死亡的威胁，那么胃癌晚期不能手术还有别的方法吗?
　　胃癌晚期有哪些症状表现：
　　1、胃癌可能有少量出血的现象，比如消化道出血。如果有呕血、黑便及大便潜血阳性，一定要尽早筛查胃癌问题。
　　2、上腹饱胀感。因此，如果日常常存在泛酸、恶心、呕吐问题的话，一定要尽早筛查胃癌问题。胃癌晚期什么症状，专家表示，如果胃癌肿瘤位于贲门，会导致进食不通畅。如果肿瘤位于胃的幽门，会导致梗阻，病人容易呕吐出腐败的隔夜食物。

　　【胃癌的高发人群】
　　1、患有癌前病变：癌前病变是指有癌变倾向的良性疾病，如：(1)慢性萎缩性胃炎，癌变率可达10%;(2)慢性胃溃疡，癌变率低于3%;(3)胃息肉，直径&2cm，多发且基底较宽者癌变率高;(4)胃部分切除者，残胃癌变率可达0.3%-10%;(5)其他癌前病变，如巨大胃粘膜肥厚症，疣状胃炎等;(6)胃镜活检病理类型： ①异形增生，也称不典型增生，由慢性炎症引起，如发展到重度不典型增生则可认为是癌前病变甚至被认为是早期癌;②胃间变则癌变机会多;③大肠型肠化生与胃癌发生关系密切。
　　2、饮食习惯不良：如饮食不规律、吃饭快速、喜高盐/热烫食品，喜食致癌物质亚硝酸盐含量高的腌制、熏制、干海货、隔夜菜，喜食烧烤的红肉、常食用霉变食物、少食新鲜蔬菜等。
　　3、长期酗酒及吸烟：酒精可使粘膜细胞发生改变而致癌变。吸烟也是胃癌很强的危险因素，青少年时期开始吸烟者危险性最大。
　　4、有胃癌或食管癌家族史：患者家属中胃癌发病率比正常人群高2~3倍。
　　5、长期心理状态不佳：如压抑、忧愁、思念、孤独、抑郁、憎恨、厌恶、自卑、自责、罪恶感、人际关系紧张、精神崩溃、生闷气等，胃癌危险性明显升高。
　　6、某些特殊职业：长期暴露于硫酸尘雾、铅、石棉、除草剂者及金属行业工人，胃癌风险明显升高。
　　7、地质、水质含有害物质：地质为火山岩、高泥炭、有深大断层的地区，水中Ca/SO4比值小，而镍、硒和钴含量高。火山岩中含有较高含量的3，4苯并芘，泥炭中有机氮等亚硝胺前体含量较高，易损伤胃粘膜。硒和钴也可引起胃损害，镍可促进3，4苯并芘的致癌作用。
　　8、幽门螺旋杆菌(Hp)感染：有研究称约半数胃癌与幽门螺杆菌感染有关。国人约60%感染该菌，但仅0.03%的人群患胃癌。
